If you have lost, damaged or misplaced your SILKS Membership Card please fill in the form below to request a replacement.

Please note there may be a £3.29 administration and postage fee that will need to be paid in order to process a replacement card.

Furthermore, the replacement card will be an identical replica of your initial membership card with the same:

- Name

- Membership Level

- Expiration Date

Once you have completed the form below you will receive an email from our team within three working days and may be asked to pay a small fee (stated above) to cover both the administration and postage cost.
